چکیده

AMYLOIDOSIS is a disease characterized by deposition of amyloid proteins in various body tissues. The natural history of primary amyloidosis is poorly understood, and the clinical diagnosis is often not made until the disease is far advanced. There is a high prevalence of conduction disturbances associated with this disease, and heart-related problems are a frequent cause of death for patients with primary amyloidosis. Sudden death is common, occurring in one third of these patients. Here we present a case of unexpected postoperative cardiac arrest in a patient who was found at autopsy to have primary amyloidosis.

Ventricular Fibrillation after Bortezomib Therapy in a Patient with Systemic Amyloidosis

A 64-year-old female was diagnosed with systemic amyloidosis associated with multiple myeloma. Bortezomib and dexamethasone-therapy was initiated; however, she developed lethal ventricular fibrillation (VF) and cardiac arrest after 84 hours of therapy. Cardiopulmonary resuscitation using direct current shocks with epinephrine and amiodarone was initiated but failed to receive cardiac function. ...

Long term results of heart transplantation in patients with amyloid heart disease.

OBJECTIVE To determine the outcome of heart transplantation for end stage amyloid heart disease in patients treated at a single centre. DESIGN Records of all patients with amyloid heart disease who underwent heart transplantation were examined to determine survival, graft involvement by amyloid, the course of systemic amyloid disease, and the cause of death. PATIENTS 10 patients, mean (SD) ...

Prognostic Significance of Doppler Measures of Diastolic Function in Cardiac Amyloidosis

Background. We have previously characterized the left ventricular diastolic filling abnormalities in cardiac amyloidosis by Doppler methods. The various filling patterns were shown to be related to the degree of cardiac amyloid infiltration. The purpose of this study was to determine the value of Doppler diastolic filling variables for assessing prognosis in cardiac amyloidosis. Methods and Res...

Catheter Ablation of Ventricular Tachycardia/Fibrillation in a Patient with Right Ventricular Amyloidosis with Initial Manifestations Mimicking Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy

Differentiating arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) from other cardiomyopathies is clinically important but challenging. Although the modified Task Force Criteria can facilitate diagnosis of ARVD/C according to clinical manifestations, histopathological examination plays a pivotal role in excluding other diseases that can mimic ARVD/C. Here, we report a patient wi...

Intracardiac thrombosis and anticoagulation therapy in cardiac amyloidosis.

BACKGROUND Primary amyloidosis has a poor prognosis as a result of frequent cardiac involvement. We recently reported a high prevalence of intracardiac thrombus in cardiac amyloid patients at autopsy. However, neither the prevalence nor the effect of anticoagulation on intracardiac thrombus has been evaluated antemortem. METHODS AND RESULTS We studied all transthoracic and transesophageal ech...
